Player Transfers
If you have moved within the CPMHA boundaries (aren’t sure? Check here) and your child has played hockey in another association, please proceed with a transfer request based on your personal scenario.
Scenario 1: Moves to the CPMHA boundaries
Players who move to Carleton Place but who have played with another association must submit the following:
1. A completed HEO Attestation of InterBranch/Association Residency Form
2. Proof of Residency:
i) A copy of the Purchase and Sale Agreement OR Lease Agreement for the new residence (1-year minimum lease), plus
ii) Any two of the following documents:
• School Registration
• Utility Bill or Cable/Internet bill (no cell phone) that matches new address
• Driver’s License (parent) that matches new address
• Vehicle insurance/registration that matches new address.
Scenario 2: Players who live outside the boundaries of CPMHA but within District 4 and wish to play with CPMHA
Players who wish to play for CPMHA but live outside the boundaries need to complete a House League Transfer Form .
This is a year by year transfer form, the reasoning needs to be justified and there is no guarantee the transfer will be approved from either association.
**Please note that registration cannot be processed until the registrar receives all correctly completed documents.
